INSTRUCTIONS

Use the surveys on this page to help you move towards your goal. Your coach will assign them to you as you work through your action plan.

1. Click on the link below the Survey Name that you want to take. The survey will open in a new window.

2. Enter your personal information at the begining of the survey.

3. Make sure to put in your business coaches email address where it ask so your coach will get a copy of what you submitt.

4. On the longer surveys you may save and come back to finish later.

5. NOTE: If you are doing the surveys without a coach, put your email address where it ask for the Biz Coach Email so you will receive a copy.
